Nomination Form for 2014 Outstanding Clinicians Awards

Research has shown that the quality of the therapeutic relationship has the greatest impact on patient outcomes in addiction treatment. Addiction Professional is pleased for the sixth year to honor clinicians who address professional challenges head-on to provide effective care for persons with addictive disorders.

The 2014 Outstanding Clinicians Awards recognize clinical professionals at four levels (counselor, clinical supervisor, medical director, and nurse), either in organizations or in private practice, who are making a difference in their programs and communities. To nominate someone for an award, please fill out the form below to submit your nomination by May 16, 2014. If you have questions, e-mail Editor Gary Enos at genos@vendomegrp.com.

Describe the nominee's work and significant accomplishments in 300 words or fewer. Possible areas to cover could include, but are not limited to, the nominee’s use of evidence-based treatments, his/her work with special populations, and his/her innovative efforts to engage clients in treatment.

Winners will be featured in the July/August 2014 issue of Addiction Professional and will be recognized formally at the National Conference on Addiction Disorders (NCAD) in St. Louis, this August.
